During the COVID-19 Crisis We are still open for your healthcare needs.

For your safety, Park West Health System has adjusted our operations during the COVID-19 State of Emergency:

Only patients with prior approval for face-to-face visits, including pregnant patients and children under 2 who need check-ups with vaccines. Only 1 healthy adult may accompany a pediatric patient.

All individuals entering the building (whether patient or "visitor") will have their temperature checked and be screened for COVID symptoms. We can not accept patients or visitors with COVID symptoms.

Worried about whether you might have been exposed, or need advice about possible COVID? Please call and we will do our best to connect you to a provider on the same day if possible! We are not testing for coronavirus at our clinics, but with a telehealth visit you can be evaluated & we can work with you to help you get testing if needed.

Sliding Fee Discount Program

Park West Health System is an affordable Community Health Center. We provide services
regardless of your ability to pay in full for services rendered.
Park West Health System has a policy for providing discounted health services to those
patients who are uninsured or otherwise unable to pay in full for services.
 

Sliding Fee Scale Discount Program

A "Sliding Fee Scale" means that we can adjust the cost of your office visits, medications and lab fees.

Sliding fees are based on household income and family size.

 

To apply you must:

  • Complete the Sliding Fee Scale application
  • Present a valid ID
  • 2 letters or bills as proof of address
  • Proof of household income (1 of the following)
    • A copy of your current year tax return
    • Copies of all paystubs for a one month period
    • Letter from your employer (On letter Head)
    • Notorized letter from employer
    • Unemployment letter from DOL or Award letter from Social Security or DSS
