Only patients with prior approval for face-to-face visits, including pregnant patients and children under 2 who need check-ups with vaccines. Only 1 healthy adult may accompany a pediatric patient.

Federally Qualified Community Health Center located in Northwest Baltimore seeking a Full-time or Part-time Board certified general pediatrician. Weekday Schedule. 2 years clinical experience in private practice or a community health setting required. No hospital inpatient covering. MD License.  Excellent benefits.

The Medical Case Manager reports to the Client Services Coordinator and provides medical case management/treatment adherence services for The Hidden Garden Program as instructed.

 

  •  Duties:
    • Set up initial interview with client;
    • Confirm Ryan White eligibility and applicable documentation;
    • Refer client to primary medical care;
    • Complete HIV/AIDS Bio-psychosocial Clinical Assessment form;
    • Determine level of case management assistance required to achieve goals and self-sufficiency (limited, intermediate, intensive);
    • Develop a care plan with the client that identifies goals, interventions, and timeframes for resolution;
    • Document progress made toward achievement of goals in chart/CAREWare;
    • Provide support, advocacy, consultation, and crisis intervention to the client and other family and household members;
    • Maintain face to face, telephone, and other encounters with and on behalf of the client based on the level of care required;
    • Follow up on scheduled and kept primary care appointments to ensure labs are updated every three-four months;
    • Provide assistance in scheduling specialty care appointments and follow up as needed;
    • Update bio-psychosocial assessment, care plan, eligibility information: insurance, income, residence every six months;
    • Participate in multidisciplinary team meetings to discuss clients with co-morbid conditions;
    • Participate in the completion of monthly, quarterly, semiannual and annual Ryan White reports;
    • Provide assistance in developing and implementing quality management initiatives;
    • Facilitate support and skills development groups; 
    • Assist in developing community partnerships for clinical support, referrals and linkage to care;
    • Attend relevant meetings and required trainings;
    • Other duties as assigned.

 

  • Qualifications:
    • Master’s Degree in social work;
    • Relevant experience and sensitivity to HIV/AIDS and African-Americans and other minority community related issues.
    • LMSW licensure;
    • An awareness of local AIDS services and resource providers at the public and community levels.

Park West Health Systems, a network of Federally Qualified Community Health Centers in northwest Baltimore City, seeks an experienced NP or MD (Internal Medicine or Family Medicine) with a heart for community medicine and public health to join its multidisciplinary care team as a primary care provider.

Park West has deep roots in the urban community it serves & seeks a primary care provider with interest in, and passion for, the treatment of prevalent community health challenges including diabetes, hypertension, substance use disorder and HIV infection. The ideal candidate would be a clinician with experience providing culturally competent substance use disorder care including medication-assisted therapy (eg. an X waiver or willingness to obtain one upon hire.) Preferred qualifications including experience with PEP and PrEP, AAHIVM certification, & love of working in a fast-paced, diverse clinical environment and forging meaningful relationships with patients and colleagues. Park West Health Systems, a network of Federally Qualified Community Health Centers in northwest Baltimore City, seeks an HIV Specialist (experienced NP or MD) to serve as the new clinical & administrative lead for its HIV Services. Park West has deep roots in the community it serves, including a robust multi-disciplinary HIV program with outreach workers, case managers, addiction services, and prison re-entry coordinator.

In addition to providing clinical care as a primary care provider, the Clinical Lead for HIV Care services would be responsible for the development of policies and procedures related to HIV standards of care & implementing these services across our 3 locations, collaborating with HIV support staff to ensure ongoing effectiveness of services, working with the Medical/Data Coordinator on outcomes and quality initiatives, and representing Park West on relevant extramural initiatives, e.g. committees of the Greater Baltimore HIV Health Services Planning Council.

The ideal candidate would be an individual with minimum 2 years experience providing clinical care to people living with HIV (NP with AAHIVM certification, MD with AAHIVM certification or ID fellowship training), a clinician with experience providing culturally competent substance use disorder care including medication-assisted therapy (eg. an X waiver or willingness to obtain one and prescribe buprenorphine when indicated), knowledgeable about PEP and PrEP, love of working in a fast-paced, multi-disciplinary environment and forging meaningful relationships with patients and colleagues. Spanish speakers are especially welcome to apply.
