Social Worker needed to ensure approporiate services and coordinate delivery of services, advocate on client's behalf, assist prison outreach coordinators and develop and complete psychosocial assessments. Must have knowledge of HIV or co-occuring substance abuse, mental illness, and homelessness to provide ongoing social work case management to HIV positive patients. Must be able to work as a part of a multidisciplinary team and have own transportation.
